As the Clinical Care Coordinator, you will need:
- Experience in quality or management within a service context.
- Up-to-date knowledge of aged care and the reforms happening in the sector.
- Strong leadership, organisational, and problem-solving skills, with the ability to manage multiple tasks and priorities.
- Excellent communication, interpersonal, and customer service skills, with the ability to build rapport and trust with residents, families, and staff.
- Demonstrated ability and willingness to work in a team and independently.
- Proficiency in Microsoft Office, particularly Word, Excel and Outlook.
- RN with current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Minimum three (5) years’ experience in residential aged care
- Broad knowledge of the Aged Care Industry and current Aged Care Act requirements
- Sound understanding of care assessment in the aged care sector and use of AN-ACC
- Demonstrated effective interpersonal and communication skills (written and verbal)
- You will have good working knowledge and understanding of AN-ACC funding and aged care standards.
Main Duties of the Role:
- Manage all clinical services in line with Our process and policy
- Advocate the implementation of continuous improvement activities in all aspects of clinical and lifestyle services
- Ensure all clinical documentation is completed accurately and on time
- Coordinate and support all scheduled audits and clinical indicators completed in line with the schedule
- Ensuring resident health records and clinical documentation is relevant and up to date
- Ensuring “best practice” medication management guidelines are observed and regularly monitored
- Contributes to the efficient and effective management of human, material and fiscal resource management according to the strategic plan and budget in line with the Mission and Philosophy of Seventh-day Adventist Aged Care (Vic) Ltd.
- Provides overall clinical monitoring such as wound management, palliative care, diabetic management, infection control and medication management.
- Maintain timely and accurate care planning, evaluations and care consultations as allocated.
- Supervises, supports and encourages staff to perform their roles and responsibilities in accordance with duties lists and residents care plans.
- Responsible for maintaining the continuity of care by follow up communication from previous shift and undertaking regular checks of the facility.
- Ensures rights and responsibilities of residents are provided and follows up concerns raised by residents or their families.
- Coordinates continuous improvement activities relevant to resident care.
- Human resource management including recruitment, selection, appointment, rostering, staff development and performance management.
- Maintains the role as the IPC lead and is responsible for the infection control protocol in the facility.
